I confirm that I am at least 18 years of age, or I am the parent or legal guardian of the participant. If the participant is a minor (under 18 years of age), I understand that a parent or legal guardian must be physically present during the entire session and must actively consent to all aspects of this research protocol. I accept full responsibility for ensuring compliance with these age and guardianship requirements.

I acknowledge that the research methodology, underlying process, techniques, frameworks, and all associated materials are proprietary and strictly confidential intellectual property owned exclusively by the company. I solemnly agree to maintain absolute confidentiality of all information disclosed during this study. I shall not, under any circumstances, disclose, publish, distribute, reproduce, explain, teach, or make available any details of the protocol, technique, methodology, or derivative information without prior written consent from the company. This obligation survives termination of my participation and extends indefinitely.

I understand and agree that I am solely and fully responsible for maintaining confidentiality of all proprietary information. I agree to be held strictly liable for all damages, costs, losses, and expenses—including attorney fees, legal proceedings, and reputational harm—arising from any unauthorized disclosure, sale, distribution, reproduction, teaching, or breach of confidentiality. The company retains the right to pursue both civil and injunctive remedies for any violation. I further agree that the company may seek specific performance, injunctive relief, and liquidated damages for breaches of this confidentiality obligation. I shall indemnify and hold harmless the company from all claims, damages, and costs arising from my breach.

I expressly agree to actively protect the company's intellectual property and shall take all reasonable measures to prevent disclosure or misuse. I understand that the company's proprietary methodology is a valuable trade secret protected by law. I agree not to photograph, record, copy, or document any materials, techniques, or processes without explicit written authorization. I commit to immediately reporting any unauthorized use, disclosure, or violation to the company. I acknowledge that violation of these IP protections may result in immediate legal action, including cease and desist letters and litigation. I understand that damages for IP theft can be substantial and include lost profits, punitive damages, and attorney fees.

I consent to the audio and/or video recording of my session for quality assurance, research, and training purposes. I understand that recordings will be stored securely and handled in accordance with applicable privacy regulations.

I understand that the company will take reasonable measures to protect my personal information and privacy, including safeguarding my location data and last name. However, I acknowledge that no security is guaranteed. The company will not spam, sell, or disclose my contact information to third parties. Exception: If I voluntarily reveal personal information, my location, or my full name in session recordings, the company may include such information in materials derived from the recordings. I am responsible for controlling what personal information I disclose verbally during sessions.

I hereby irrevocably transfer all intellectual property rights, copyright, and exclusive ownership to the company, including but not limited to: all resulting footage, recordings, derivative works, edited materials, transcripts, data, and any content derived from my participation. I acknowledge that the company owns these materials in perpetuity worldwide. I waive all moral rights and claim no ownership or royalties. I grant the company perpetual, worldwide, exclusive, and irrevocable rights to edit, reproduce, distribute, and commercialize all materials. I understand that the company's intellectual property is protected by law, and that I retain no rights to any materials produced during or after my participation.

I understand that the results of this research are not guaranteed. I further acknowledge that the company and its agents, including researchers and practitioners, shall not be held liable for any outcomes, results, or adverse effects resulting from my participation in this study, unless arising from gross negligence or willful misconduct. I understand that this research protocol is not therapy and may not be facilitated by licensed medical doctors, neuroscientists, or psychological professionals. I acknowledge that this is a research study and not a substitute for professional medical, psychological, or psychiatric care. I am solely responsible for seeking appropriate professional care as needed.

I agree that I will not solicit personal information from the facilitator, attempt to contact them outside the scope of this research study, or hold the facilitator or the company liable for any reason whatsoever. I understand that the facilitator-participant relationship is strictly professional and limited to the research protocol. I accept full responsibility for my participation in this study, the results I experience or do not experience, and any disappointments that may arise from my involvement.

Should a dispute arise between me and the company or any of its agents regarding this research study, I agree that I will first seek resolution through mediation with a licensed facilitator before pursuing any legal action or attorney involvement in the state of California. I acknowledge that this dispute resolution process is mandatory before any litigation may be initiated.
