If the participant is a minor (under 18 years of age), I understand that a parent or legal guardian must be physically present during the entire session and must actively consent to all aspects of this research protocol. I accept full responsibility for ensuring compliance with these age and guardianship requirements.

I acknowledge that the research methodology and underlying process are proprietary and confidential. I agree to maintain the confidentiality of all information disclosed during this study and shall not disclose, publish, or distribute any details of the protocol without prior written consent from the company.

I understand and agree that I am solely responsible for maintaining the confidentiality of this research. I agree to be held liable for any damages arising from unauthorized disclosure, sale, distribution, or breach of confidentiality of the proprietary information or methodology shared during this study.

I consent to the audio and/or video recording of my session for quality assurance, research, and training purposes. I understand that recordings will be stored securely and handled in accordance with applicable privacy regulations.

I understand that the company will take reasonable measures to protect my personal information and privacy, including safeguarding my location data and last name. However, I acknowledge that no security is guaranteed. The company will not spam, sell, or disclose my contact information to third parties. Exception: If I voluntarily reveal personal information, my location, or my full name in session recordings, the company may include such information in materials derived from the recordings. I am responsible for controlling what personal information I disclose verbally during sessions.

I hereby transfer all intellectual property rights, including but not limited to the resulting footage, recordings, and any derivative works, to the company for its exclusive use. I grant the company the perpetual, non-exclusive right to use my likeness and first name in connection with this research, materials, and documentation.

I understand that the results of this research are not guaranteed. I further acknowledge that the company and its agents, including researchers and practitioners, shall not be held liable for any outcomes, results, or adverse effects resulting from my participation in this study, unless arising from gross negligence or willful misconduct. I understand that this research protocol is not therapy and may not be facilitated by licensed medical doctors, neuroscientists, or psychological professionals. I acknowledge that this is a research study and not a substitute for professional medical, psychological, or psychiatric care. I am solely responsible for seeking appropriate professional care as needed.

I agree that I will not solicit personal information from the facilitator, attempt to contact them outside the scope of this research study, or hold the facilitator or the company liable for any reason whatsoever. I understand that the facilitator-participant relationship is strictly professional and limited to the research protocol. I accept full responsibility for my participation in this study, the results I experience or do not experience, and any disappointments that may arise from my involvement.

Should a dispute arise between me and the company or any of its agents regarding this research study, I agree that I will first seek resolution through mediation with a licensed facilitator before pursuing any legal action or attorney involvement in the state of California. I acknowledge that this dispute resolution process is mandatory before any litigation may be initiated.
